Evidence

Clinical credibility should be visible, inspectable and independent.

What IntractMD is built on, how it reaches a result, and what it does not claim to do.

Data sources

Medication content is grounded in FDA-approved prescribing information, with drug terminology standardized using NLM RxNorm. Patient-facing explanatory material draws on MedlinePlus. Source and version information is recorded with the underlying data.

Interaction methodology

Drug-drug, drug-supplement and drug-food interactions are evaluated together rather than in separate passes. Interaction content is pre-computed against FDA labeling, graded by clinical severity, and presented with the finding that produced it.

Polypharmacy methodology

A 28-factor risk framework feeds the Composite Polypharmacy Risk Score across eight pharmacological dimensions. CPRS is a decision-support signal, not a validated clinical instrument or an outcome-prediction model, and is not presented as one.

Deprescribing methodology

Candidates are identified from AGS Beers Criteria®, STOPP/START v3 and CMS Star Ratings medication-safety measures, ranked by the strength of the published recommendation and shown with their source. Rescue and replacement therapy are excluded from candidacy. Output is a prompt for pharmacist review, never a recommendation to discontinue.

AI and clinical decision support

AI-assisted analysis synthesizes findings across a regimen and identifies potential risk patterns. Every analysis is anchored to authoritative source data first. Findings are labeled as established, computed or predictive, and a predictive finding is never presented as an established clinical fact.

Independent clinical review

Clinical content is undergoing independent PharmD review, covering interaction analysis, supplement and OTC coverage, CPRS methodology, deprescribing safeguards and patient-facing language in both English and Spanish. Reviewer credentials, scope, findings and date will be published here on completion.

Clinical evidence

Clinical evidence and methodology

IntractMD is built around established medication-safety information and clinical concepts relevant to polypharmacy, drug interactions, anticholinergic burden and deprescribing.

Clinical methodology

Our methodology describes how medication information is evaluated, how risk signals are generated, and how results are presented for clinical review.

Evidence sources

  1. CMS, FDA, NIH/NLM, AHRQ, CDC and other authoritative government sources where applicable.
  2. Peer-reviewed literature and established clinical journals.
  3. Recognized professional organizations and guideline bodies.
  4. Specialized or commercial references where they are the authoritative source for a specific dataset or service.

Independent clinical review

Resolve Medical, LLC supports independent clinical review of IntractMD's medication-interaction analysis, OTC and supplement coverage, anticholinergic and CNS methodology, CPRS, deprescribing safeguards and patient-facing language. Independent review findings will be presented separately when available.

CMS / Star Ratings

Medication safety measures are now part of the 2027 Star Ratings.

CMS added two new Part D measures beginning with the 2027 Star Ratings: Concurrent Use of Opioids and Benzodiazepines (COB) and Polypharmacy: Use of Multiple Anticholinergic Medications in Older Adults (Poly-ACH). Both are process measures with a weight of one.

Separately, the Medication Therapy Management (MTM) Program Completion Rate for Comprehensive Medication Review measure was moved to the CMS display page for measurement years 2025 and 2026. CMS states that it will return to the Star Ratings as a new measure beginning with the 2029 Star Ratings, using 2027 measurement-year data.

How IntractMD Supports CMS-Related Medication Safety Workflows

IntractMD can support medication-review and medication-safety workflows relevant to these areas, but it does not determine CMS measure performance, establish payment eligibility, or guarantee an improvement in a plan's Star Rating. CMS specifications and annual technical documentation remain the authoritative sources.
