Medicare Quality Measures Spotlight Medication Safety — IntractMD™ Identifies Hidden Supplement Risks

CLEVELAND, Ohio — August 11, 2026 — Millions of older Americans take prescription medications together with vitamins, herbal supplements and over-the-counter products. Yet during Medicare Comprehensive Medication Reviews (CMRs), identifying those non-prescription products often depends on what patients can remember during a single conversation.

That gap is becoming more important.

Beginning with the 2027 Medicare Star Ratings, two new Part D quality measures place greater emphasis on medication safety in older adults, while Medicare has already nearly doubled the number of beneficiaries eligible for annual Comprehensive Medication Reviews. Together, those changes increase both the volume of reviews and the importance of identifying everything a patient is taking — not just prescription medications.

Why this matters now

Several Medicare changes are already affecting organizations today.

Effective January 1, 2025, the Centers for Medicare & Medicaid Services (CMS) expanded eligibility for Part D Medication Therapy Management (MTM), increasing the number of beneficiaries eligible for annual Comprehensive Medication Reviews from approximately 3.6 million to roughly 7.1 million.

At the same time, CMS is calculating performance for two Pharmacy Quality Alliance measures that become part of the 2027 Medicare Star Ratings:

  • Concurrent Use of Opioids and Benzodiazepines
  • Polypharmacy: Use of Multiple Anticholinergic Medications in Older Adults

Performance for those measures is based on 2025 measurement-year data, meaning organizations are already being evaluated.

Separately, the MTM Program Completion Rate for Comprehensive Medication Review moves temporarily to the CMS Display Page for the 2025 and 2026 measurement years before returning as a Star Ratings measure beginning with the 2029 ratings using 2027 performance data.

The challenge isn’t prescription drugs — it’s everything else

CMS defines a Comprehensive Medication Review as an annual review of a beneficiary’s complete medication regimen, including prescription medications, over-the-counter medications, herbal products and dietary supplements.

Prescription medications are typically available through pharmacy claims and electronic health records. Supplements, vitamins and many over-the-counter products are not.

As a result, pharmacists frequently rely on patient recall to identify products that may meaningfully affect medication safety.

That becomes increasingly important when evaluating cumulative medication burden. For example, commonly purchased over-the-counter allergy medications may contribute to anticholinergic burden even though they may never appear in a prescription record.

The missing piece is supplements
“Every health plan and health system we speak with already has prescription drug interaction checking,” said Tom Lash, Founder of Resolve Medical.

“What they don’t have is a practical way to consistently capture the supplement layer. If pharmacists don’t know a patient is taking supplements or certain over-the-counter products, or don’t have an immediate prompt while reviewing the patient’s medication regimen, they can’t fully evaluate medication risk. We didn’t just build a SaaS subscription to a database. We built a broader four-surface platform that fits the existing pharmacy workflow — and it looks different depending on whether you are the pharmacist in the encounter, the case manager working a panel, or the plan looking at a population.”
What IntractMD does

IntractMD screens prescription medications together with dietary supplements, vitamins and foods in a single query, prioritizing clinically actionable risks while producing a Composite Polypharmacy Risk Score (CPRS) across eight clinical dimensions, including bleeding risk, cardiac risk, central nervous system effects, CYP450 interactions and renal/hepatic considerations.

The platform draws on a precomputed knowledge base containing more than 173,000 drug-pair analyses covering approximately 590 commonly prescribed U.S. medications. Drug terminology is standardized using RxNorm, and interaction content is grounded in FDA-approved prescribing information. Results are available in both English and Spanish and can be shared directly with patients following a medication review.

Resolve Medical delivers IntractMD across four connected applications designed for different users within the medication management workflow:

  • A patient-facing interaction checker for use before or after appointments.
  • A proactive population-screening engine for health plans and pharmacy organizations.

CLEVELAND, Ohio — July 28, 2026 — Resolve Medical, LLC today launched IntractMD™, an AI-powered medication interaction platform that screens a patient’s prescriptions together with the dietary supplements, vitamins, and foods they take — a category of risk largely invisible to claims data and electronic health records.

Adverse drug events cause roughly 1.5 million emergency department visits and 500,000 hospitalizations in the United States each year, at an estimated $3.5 billion in excess medical costs. Older adults carry the heaviest share: 34.5% of those emergency visits involve adults 65 and over, and 43.6% of them end in a hospital admission. Supplement use is widespread in exactly that group — roughly 85% of adults over 50 take at least one — yet FDA-cited estimates suggest up to 70% never tell a healthcare provider.

“A pharmacist can see every prescription a patient fills at their counter and still may miss other meds filled elsewhere, or supplements and foods that potentially interfere with the efficacy of the prescription being filled,” said Tom Lash, Founder of Resolve Medical. “That information isn’t in any claims feed or medication list. IntractMD draws on a pre-computed database of 124,750 drug-pair analyses covering the 500 most-prescribed medications in the United States and produces output in both English and Spanish. The platform is the subject of a U.S. provisional patent application filed in June 2026.
